Stimulates osteogenesis, restores a positive balance between bone resorption and restoration, increases bone mineral density (regulates phosphorus-calcium metabolism), promotes the formation of bone tissue with a normal histological structure.

Pharmacokinetics

Suction

The bioavailability of alendronate at a dose of 70 mg when taken orally on an empty stomach 2 hours before a standard breakfast was 0.64% in women and 0.59% in men. When taken 1 hour or 30 minutes before breakfast, the bioavailability of alendronate decreased to 0.46% and 0.39%, respectively. In clinical studies, the effectiveness of alendronate has been confirmed when taken at least 30 minutes before the first meal or drink. The bioavailability of alendronate is negligible when taken with food or within 2 hours after a meal. When taken together with coffee or orange juice, the bioavailability of the drug is reduced by approximately 60%. The concentration of the drug in the blood is negligible (less than 5 ng / ml).

Distribution

Alendronate after intravenous administration at a dose of 1 mg / kg is temporarily distributed into soft tissues and then quickly redistributed to bone tissue or excreted in the urine. Metabolism

There is no evidence that alendronate is metabolized in humans.

Indications

- treatment of osteoporosis caused by long-term use of corticosteroids.

Contraindications

- strictures or achalasia of the esophagus and other conditions that slow down the movement of food through the esophagus;

- inability of the patient to stand or sit for 30 minutes;

- hypocalcemia;

- severe renal failure (CC less than 35 ml / min);

- severe violations of mineral metabolism;

- pregnancy;

- lactation period (breastfeeding);

- children's age (efficacy and safety of use have not been established);

- hypersensitivity to alendronate or other components of the drug.

Carefully: diseases of the gastrointestinal tract, such as dysphagia, gastritis, diseases of the esophagus, duodenitis, peptic ulcer in the acute phase, active gastrointestinal bleeding or surgery in the upper gastrointestinal tract during the year preceding the start of treatment; hypovitaminosis D.

Dosage

The drug should be used to ensure the daily requirement of calcium and D.

The optimal duration of the drug has not been established. The need for continued bisphosphonate therapy should be reassessed on a regular basis, especially after 5 or more years of use.

To ensure proper absorption of the drug Foroza tablets should be taken in the morning on an empty stomach, at least 30 minutes before the first meal, drink or other medication, with a glass of plain water (at least 200 ml). Other drinks (including mineral water) may reduce the absorption of the drug.

Side effects

In a one-year study in postmenopausal women with osteoporosis, the overall safety profiles of alendronic acid once a week at a dose of 70 mg (n=519) and at a dose of 10 mg/day (n=370) were similar.

Overdose

Symptoms: abdominal pain, dyspeptic disorders, dysphagia, heartburn, esophagitis, gastritis; hypocalcemia and hypophosphatemia may develop.

Treatment symptomatic. The use of milk and antacids to bind alendronate is shown. Due to the risk of damage to the esophagus, vomiting should not be induced, the patient should be in an upright position.

drug interaction

Simultaneous use of calcium preparations (including nutritional supplements), and certain other drugs for
ingestion, as well as food and liquid intake (including mineral water) impairs the absorption of alendronic acid. In this regard, it is recommended to take other drugs no earlier than 30 minutes after using the drug Foroza. NSAIDs (including) can increase the side effects of alendronic acid on the gastrointestinal mucosa.

In clinical studies in patients taking preparations containing estrogen (intravaginally, transdermally or orally) concomitantly with alendronate, no clinically significant interaction was found.

Although no specific drug interaction studies have been conducted, the use of alendronate in clinical trials with a large number of widely used drugs has not been associated with the development of a clinically significant interaction.

Alendronate can cause local irritation of the mucous membrane of the upper gastrointestinal tract. Foroza should be used with caution in patients with exacerbation of upper GI diseases such as dysphagia, esophageal disease, gastritis, duodenitis, ulcers, active gastrointestinal bleeding, surgery in the upper GI tract during the year preceding the start of treatment, with the exception of pyloroplasty, tk. the use of the drug can lead to a deterioration in the course of the underlying disease.

Treatment decisions must be made on a patient-by-patient basis after a careful risk/benefit assessment, especially in patients with Barrett's esophagus.

In the treatment of alendronate, cases of adverse reactions from the esophagus (esophagitis, ulcer or erosion of the esophagus) are known, sometimes occurring in severe form and requiring inpatient treatment and in rare cases complicated
stricture formation.

It is necessary to control the possibility of any adverse reactions from the esophagus. The patient should be informed about the need to stop taking the drug and consult a doctor if dysphagia, pain when swallowing, chest pain and heartburn develop.

Rare cases of gastric and duodenal ulcers have been reported in post-marketing reports, sometimes
severe and complicated, although no increase in this risk was noted in the extended clinical trials of alendronate.

The risk of severe esophageal side effects is higher in patients taking alendronic acid in violation of the instructions in this leaflet and / or continuing to take it after the onset of symptoms indicating irritation of the esophagus. It is important to explain in detail to the patient the rules for taking the drug and make sure that he understands them. Patients should be aware of the increased risk of adverse events from the esophagus in case of deviation from the requirements of the instructions.

Correction of hypocalcemia and other metabolic disorders (such as vitamin D deficiency) is necessary before starting therapy with Forose. Due to the increase in bone mineral density during therapy with alendronic acid, a slight clinically asymptomatic decrease in serum calcium and phosphate concentrations is possible, especially in patients receiving corticosteroids, in which calcium absorption may be reduced. Therefore, it is important to ensure that sufficient amounts of calcium and vitamin D are supplied to the body, which is especially important in patients receiving corticosteroids.

Patients should be warned that if they accidentally skip taking the drug according to the 1 time per week regimen, they should take 1 tablet in the morning of the next day (it is unacceptable to take 2 tablets on the same day). In the future, you should continue to take 1 tablet on the day of the week that was chosen at the beginning of therapy.

There is also evidence of the development of osteonecrosis of the jaw in patients with osteoporosis receiving oral bisphosphonates. When assessing the individual risk of developing osteonecrosis of the jaw, risk factors such as:
bisphosphonate activity (highest in zoledronic acid), route of administration and total dose of the drug; oncological diseases, chemotherapy, radiation therapy, taking corticosteroids, taking angiogenesis inhibitors, smoking; history of dental disease, poor oral hygiene, gum disease, invasive dental procedures, ill-fitting dentures. Before starting therapy with oral bisphosphonates, patients should undergo a dental examination with appropriate preventive dental care. During treatment, these patients should, if possible, avoid invasive dental intervention. For patients who develop osteonecrosis of the jaw during treatment with bisphosphonates, dental surgery may exacerbate the condition.

Data on a possible reduction in the risk of developing osteonecrosis of the jaw after discontinuation of bisphosphonates in patients who require dental intervention are not available. In every case
the decision should be made by the attending physician based on an assessment of the ratio of the expected benefit to the possible risk for a particular patient. Patients should be educated on the importance of good hygiene during bisphosphonate therapy.

Osteonecrosis of the external auditory canal has been reported and has been predominantly associated with long-term use of alendronate. Possible risk factors for osteonecrosis of the external auditory canal include steroid use, chemotherapy, infections, and trauma.

Bone, joint and/or muscle pain has been reported in patients receiving bisphosphonates. These symptoms were rarely severe and/or disabling. The time of onset of symptoms ranged from one day to several months from the start of therapy. In most patients, symptoms resolved after discontinuation of treatment. In some patients, symptoms reappeared when the same drug or another bisphosphonate was restarted.

Patients taking alendronic acid for a long time may experience pathological (i.e., with little force or spontaneous) subtrochanteric fractures, as well as fractures of the proximal femoral shaft. Fractures can occur after minimal or no trauma. Some patients may develop hip pain, often with outward signs of stress fractures weeks/months before a complete femoral fracture occurs.

Atypical fractures of the proximal femoral shaft were often bilateral, so patients
with a long-term fracture of the shaft of the femur, taking bisphosphonates, the examination of the opposite hip should be performed. It is known that the fractures described do not heal well. Discontinuation
bisphosphonates in patients with stress fractures, it is advisable to carry out after assessing their condition based on an individual assessment of the risk/benefit ratio.

During post-marketing use, there have been rare reports of severe skin reactions, including Stevens-Johnson syndrome and Lyell's syndrome (toxic epidermal necrolysis).

Other causes of osteoporosis, in addition to estrogen deficiency and age, should be taken into account.

Influence on the ability to drive vehicles and mechanisms

Studies on the effect of alendronic acid on the ability to drive vehicles and
work with mechanisms was not carried out. However, since against the background of taking alendronic acid, dizziness may develop, as well as other side effects, care should be taken when driving vehicles, mechanisms and refrain from performing these activities in case of side effects.

Pregnancy and lactation

Foroza is contraindicated during pregnancy and during breastfeeding.

There are no data on the effect of alendronic acid on the fetus when using the drug during pregnancy. However, there is a theoretical risk of negative effects on the fetus (especially bone tissue) if pregnancy occurs after a course of bisphosphonate therapy. Once ingested, bisphosphonates are incorporated into the bone matrix, from where they are gradually released over several years. The amount of bisphosphonates embedded in the bone matrix and able to get back into the systemic circulation directly depends on the dose and duration of the drug.

In animal studies, disturbances in the formation of fetal bone tissue have been identified with the use of high doses of alendronic acid and dysfunction of labor activity associated with hypocalcemia.

It is not known whether alendronic acid passes into breast milk, so the use of the drug during breastfeeding is contraindicated.

The founder of this park was the merchant Alexander Grigoryevich Kuznetsov. This is a man who made tea accessible to all segments of the population, significantly reducing its price for sale.

In the village of Foros in the Crimea, Kuznetsov built himself a cozy house, where tours are now held, and also laid out a magnificent garden with exotic varieties of various plants and trees. There are currently 200 plant species.
